# Premier Assist booking > Urgent care at home in New York and New Jersey: what a visit is, and how to request one, for people and for AI assistants. ## What a visit is A licensed nurse or technician comes to your address with equipment. They take vitals, examine, run point-of-care tests and draw blood. A physician assistant joins by video during the same visit, working under a supervising physician. The on-site clinician holds the connected instruments, so the PA examines rather than watches, diagnoses, and sends any prescription to the pharmacy before the visit ends. No physician comes to the house. Visits usually take about 20 minutes. ## Where New York and New Jersey, in these areas only: Nassau County on Long Island (the Five Towns, Long Beach, Oceanside, Rockville Centre, Hempstead, Great Neck, Hicksville and the rest of the county), Far Rockaway in Queens, and Midwood, Gravesend, Homecrest, Sheepshead Bay and Manhattan Beach in Brooklyn (ZIP codes 11223, 11229, 11230 and 11235); and in New Jersey, Ocean County (Lakewood, Toms River, Jackson, Brick, Manchester, Lakehurst and the shore towns down to Tuckerton) and Long Branch, West Long Branch, Deal, Allenhurst, Oakhurst and Eatontown in Monmouth County (ZIP codes 07711, 07723, 07724, 07740, 07755 and 07764). Not the rest of New York City, Suffolk County or Monmouth County. The booking page checks the visit ZIP code and says whether that address is covered. If it is not, leave an email and we will write when the area opens. ## Hours - Usual office hours: 9:00 AM to 8:00 PM, Sunday through Friday. Closed Saturday. - Hours and closed days change (holidays, weather) — GET /api/availability (JSON) is the live answer, and the booking page shows the same. - First visit of the day starts at 10:30 AM. The last visit starts at 6:30 PM. - A coordinator's first confirmation call of the day is at around 10:15 AM; during the day the call comes within about ten minutes of the request. - "As soon as possible" visits can only be requested while the office is open. From 7:00 PM, a request is still taken, and the coordinator's call is where we say whether a provider can still come that night. ## How to book Nothing is charged at booking. A coordinator calls to confirm the appointment over the phone, a confirmation email follows, and then the provider is dispatched. Your confirmation number looks like PA-XXXXXX. ### On the website Four steps at https://booking.premierassist.com/ 1. When. Tools: get_service_info, check_service_area, get_availability, register_interest, request_visit. - JSON API described at https://premier-assist-booking-api-bzh2h9btfhfxa5au.eastus-01.azurewebsites.net/api/openapi.json: GET /api/service-area?zip=, GET /api/availability?date=YYYY-MM-DD, GET /api/prices, POST /api/bookings. If you are booking for someone: - Run the emergency check first. If any red flag below applies, do not book. Tell them to call 911. - Call get_availability instead of assuming what times are open. - Send the insurer as printed on the card. check_service_area lists the plans billed in the visit's state and the few not accepted; a plan on neither list is still booked and a coordinator checks it, a plan not accepted means paying out of pocket. - Read the details back and get the patient's explicit consent before sending. - Send only what the booking asks for. - A bloodwork visit always includes Venipuncture, the blood draw — it is added to an out-of-pocket booking automatically. ## What you'll be asked For each patient: name, date of birth, gender, and symptoms. Then either insurance details with a photo of both sides of the card, or paying out of pocket. For the booking: a visit address with a New York or New Jersey ZIP code, a mobile number and an email. A coordinator asks for the card photo on the confirmation call. Symptom options are sore throat, fever, cough, congestion, UTI symptoms, and bloodwork. Bloodwork needs the doctor's lab order uploaded. ### Insurance Insurance is billed for these plans. A plan that is not listed can still be booked and a coordinator checks it on the confirmation call, except the few marked not accepted. Anyone can pay out of pocket: nothing is charged at booking, and the coordinator arranges payment on the call.
